    • BLS - Must be AHA, RQI is acceptable. Required for all RNs and RRT. Not required for Laboratory, Medical Technologist positions
    • Must be AHA, RQI is acceptable. Required for Peds, Surgery, PACU, ER, ICU and RRT. Preferred for OR.
    • Must be AHA, RQI is acceptable. Required for L&D and RRT.
    • Required for ICU and Tele. Preferred for ER.
    • Required as applicable
    • Must be passport-style photo: shoulders up, facing forward with a solid/plain background. No photo filters or AI photos allowed. Headphones or wireless hands-free devices are not acceptable in the photo. All facial piercings must be removed.
