Job Description:

This PRN Nurse Practitioner role involves conducting home risk assessments for adult and geriatric patients, including medical history review, health evaluation, and social factors, during visits up to one hour.
New grads are encouraged to apply; prior experience is preferred but not required.
The position offers flexible hours (days, evenings, nights, weekends) and potential earnings of $2,000-$10,000/month based on assessments completed.
Work locations include patients' homes, skilled facilities, and community settings in Palestine, TX.
The role requires a Master's degree or equivalent, current RN and NP licensure, board certification, BLS/ACLS, and travel ability.
Responsibilities include performing assessments, collaborating with healthcare teams, providing patient education, and documenting findings per protocols.
