Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. - Discharge Planning assists with LTC, SNF, Rehabilitation, Behavioral/Mental Health, Home Care, DME, transportation and other placement planning needs. - Discharge Planning assists with interactions with CPS, APS, Adoptions and end of life planning and completion of Advance Directives. - Discharge Planning is a liaison with third-party payers and government programs to access special services for patients. Qualifications: Minimum Qualifications: - ADN or BSN required. - Current licensure applicable to professional affiliation required. - Certification by the National Association of Healthcare Professionals or InterQual is desirable. - Current New Mexico licensure as a Registered Nurse. - Certification as CCM or ACM desired.
