Job Description:
SHIFT: 5 Days - 8 Hours (8am - 4:30pm)START: 11/16/2025RATE:

CertificationsCurrent American Heart Basic Life Support (BLS)card
from AHA.Current or eligible for licensure as a Physical Therapist
by the New Mexico Board of Physical Therapy.Weekend

Requirement / LocationsNoNurse / Patient Ratio1:1, various dx with
a large portion of elective and traumatic for evaluation, planning,
directing and administering physical therapy modalities of
treatment as prescribed by a licensed physician. Administers
treatments and physical agents as prescribed by a referring
physician in an effort to restore function and prevent disability
following injury, disease or physical disability. Assists patients
to reach their maximum performance and level of functioning, while
learning to live within the limits of their capabilities. The staff
therapist participates in operational aspects of the department,
maintains performance improvement activities within the department
and participates in CQI activities. Participates in all infection
control, departmental equipment training, organizational safety and
fire safety programs. This description is not intended to be a

required core competencies.Complies with set Policies and
Procedures (i.e. name tag, dress code, parking, smoking,
etc.)Accurately interprets referrals resulting in a proper and
complete assessment of the patient.Gathers and interprets various
data elements from the medical record, other care providers, from
patient and family resulting in an effective plan of care
(POC).Proactively communicates with other physical therapists when
the presentation of the patient exceeds one s skill, knowledge, or
understanding of the patient s condition.Implements POC that
attains expected clinical outcomes.Understands the parameters of
patient s condition each visit and adjusts behaviors
accordingly.Completes all documentation thoroughly and charges
accurately the same day treatment is rendered.Develops in
conjunction with the patient and family a set of realistic,
reasonable goals with associated time frames and
frequencies.Communicates effectively the POC to delegates of care
in a manner that is easily understood and easily translated into
action.Monitors the established POC and goals and modifies them as
indicated based on the patient s response to treatment.Utilizes
input from supervised staff and other care providers modifying POC
or treatment according to patient s condition.Recognizes cognitive,
physiological, physical, and functional changes occurring with the
patient and modifies POC and/or communicates with other care
providers as indicated.Proactively communicates in a timely manner
with the patient s physician and/or nursing staff when changes in
orders are appropriate.Monitors the results of the communication to
ensure that the desired outcome is achieved.Establishes and
maintains patient and family relationships to maximize clinical
outcomes and eventual disposition.Actively plans for the patient s
discharge from the day of admission through the entire length of
stay demonstrated in patient documentation and participation in
meetings or discussions with other care providers.Efficiently
utilizes the time and skills of supervised staff that maximizes
productivity.Treats PTA and PT Techs with respect at all
times.Ability to observe and evaluate treatment effect, recommends
change to physician as needed.Ability to perform an appropriate
assessment on all patients as related to the therapy requested and
provided and reassessments as per policyAble to assess patient pain
interfering with optimal level of function or participation in
rehabilitation; makes appropriate physician contact for
intervention.Performs all aspects of patient care in an environment
that optimizes patient safety and reduces the likelihood of
medical/health care errors.Formulates and implements a
teaching/education plan based upon identified learning needs of the
patient and family and evaluates effectiveness of learning on an
ongoing basisDemonstrates knowledge of physical therapy
modalities.Treats patients and their families with respect and
dignity. Identifies and addresses psychosocial, cultural, ethnic
and religious/spiritual needs of patients and their
families.Manages and operates equipment safely and correctly.
Maintains department cleanliness and safety.Communicates
appropriately and clearly with physicians and other members of the
patient care team.Works with Case Management to facilitate
discharge planning for patients.Participates in coordinating and
implementing patient care to ensure the patients needs are met and
hospital policy is followed.Maintains records pertinent to
personnel and operation of the department.Demonstrates an ability
to be flexible, organized and function under stressful
situations.Utilizes time in a productive manner that emphasizes
patient care, and the by-products related to delivering that School
diploma or equivalent.Certificate, Bachelors, Masters, or Doctorate
degree in Physical Therapy from an approved and accredited program
by the American Physical Therapy Associate.ExperienceAbility to
read and communicate effectively in English-additional languages
preferred.Strong written and verbal skills.Basic computer
knowledge.Knowledge of physical medicine, physical therapy
modalities, anatomy and physiology.Knowledge of physical therapy
machine operation, maintenance and repairs of same.Performs related
responsibilities as assigned. Licenses, Registrations, or
CertificationsCurrent American Heart Basic Life Support
