Job Description:
This position may qualify for a sign-on bonus. Job Summary
Coordinates the care needs of assigned patients and develops
comprehensive plans to manage care delivery across the patient care
continuum. Partners with patients and their primary physicians to
develop customized care plans based on their individual needs and
preferences. Collaborates with physicians, nurses, allied health
professionals, social work, and others to ensure appropriate tests
and treatments are delivered in a timely fashion. Advocates for the
patient. Balances care needs and financial considerations to ensure
efficient and effective treatments are achieved. Who We Are We are

Education Required: Academic degree in nursing. Preferred:
Bachelor's or master's degree. Experience Required: 2 years nursing
experience. Certifications Required: Currently licensed, certified,
or registered to practice profession as required by law, regulation
in state of practice or policy. AHA BLS. LI-AT1 Tenet complies with
federal, state, and/or local laws regarding mandatory vaccination
of its workforce. If you are offered this position and must be
vaccinated under any applicable law, you will be required to show
proof of full vaccination or obtain an approval of a religious or
medical exemption prior to your start date. If you receive an
exemption from the vaccination requirement, you will be required to
