We are seeking a detail-oriented and compassionate Intake
Coordinator to join our Early Intervention (EI) team . In this
role, you will serve as the first point of contact for families ,
guiding them through the intake and evaluation process and ensuring
a smooth transition into services. The Intake Coordinator plays a
key role in coordinating referrals, facilitating evaluations, and
supporting families through eligibility determination in alignment
with New Mexico FIT Program guidelines . Key Responsibilities
Manage incoming referrals and coordinate the intake and evaluation
process Serve as the first point of contact, educating families on
EI services, costs, and parental rights Schedule and coordinate
Comprehensive Multidisciplinary Evaluations (CME) Conduct
face-to-face meetings with families to review evaluation results
and next steps Maintain consistent communication with families
throughout the intake process Ensure compliance with EI
documentation, billing, and timeline requirements Enter and
maintain accurate family records in the database Coordinate
eligibility determination and enrollment into the EI program
Request and review medical records from providers as needed Provide
families with community resources and referrals Facilitate
transition planning for children and families Collaborate with
multidisciplinary team members to ensure high-quality service
delivery Meet program benchmarks and performance expectations
Qualifications Bachelor’s degree or higher in a field approved by
New Mexico FIT Program guidelines Experience in case management or
children’s services preferred Completion (or ability to complete)
NM FIT FSC coursework required for billing Bilingual
English/Spanish required Strong organizational, communication, and
customer service skills Detail-oriented with the ability to manage
timelines and multiple priorities Critical thinking skills and
ability to work in a dynamic environment Valid driver’s license,
reliable vehicle, and auto insurance for home visits Benefits &
